Caregivers come in all shapes, sizes, and personalities.
They may be family members, volunteers with hospitals or hospice organizations, medical professionals; or someone who works at an independent living or assisted living retirement community. Sometimes those who advocate for the elderly and disabled, who push for helpful legislation, or develop support programs are really caregivers.

Think about the caregiver in your life and select a suitable category.
Then complete the nomination form and submit the nomination. You’ll feel great the minute you take a moment to acknowledge that very special caregiver in your life.

Who is eligible?
Someone you know. Or perhaps you fit the criteria. Feel free to nominate yourself.
